NDIS Consent Form

At Discover My World ELC, we strive to give our support to families be it through our education and care at the service or our external linkages with organisations that assist in children's development!

What is NDIS (National Disability Insurance Scheme)?

​

The NDIS or National Disability Insurance Scheme is a government initiative that supports eligible people with disability, their family and carers. It is jointly governed and funded by the Australian and participating states and territory governments. 

​

The NDIS also supports families with children who have a developmental delay or disability through the Early Childhood Early Intervention (ECEI) approach. The ECEI approach supports families to help children develop the skills they need to take part in daily activities and achieve the best possible outcomes throughout their life

How does it work?

​

At Discover My World, we continuously create partnerships with individuals and organisations that would benefit the service's advocacy towards Early Childhood Development. Through this consent form, we will be contacting an Early Childhood Partner who will assist us in the process of tailoring the kind of support that your child may need based on their current developmental circumstance. 

​

The Early Childhood Partner will then provide us information about the supports and services available in our local community to help your child achieve their goals. The type of support provided is about what is best for your child and family. This might include:

​

  • Information and linkages to help you access supports and services available in your local community.

​

  • Short-term early intervention supports if this is the best way to support your child.

​

  • Where your child may require longer term early childhood intervention supports, the Early Childhood Partner can help you request access to the NDIS.

​

Once your child has access to the NDIS, the Early Childhood Partner will work with your family to develop a plan that supports your child’s goal, they will explain your plan and support you to link with your chosen providers.

Our Commitment

We will continuously work together with the Early Childhood Partner who will continuously monitor and review the progress that your child has made against the goals you have set out for them. We will support your family towards improving your child’s independence and participation in everyday activities.
